feeding baby wont take an occational bottle--help please!?

My 9 week old baby has been exclusively fed. She wont take any pacifier out there, i have tried all. Anyway, I have a sewing cl once a week and she wont take the playtex nurser bottles anymore. when she was first born, i had to substitute formula once or twice a day until my milk came in fully, which was about at 6 weeks. then maybe once a week since then she would take some bottle with a lot of fuss. I would leave her with my mom for 2 hours to go out to dinner with my husband.when i would come back she will be so hungry from not taking the bottle well. NOW she will NOT take the bottle at all! As soon as i put it in her mouth she cries. ANYONE HAVE SUGGESTIONS OR SAME SCENARIO.
